Rates

Membership fees are charged monthly via our online system:
$31.50/month for children 0-18
$63/month for adults 19-39
$70/month for adults 40-59
$75/month for adults 60+
Family rate: max of $200/month

Membership fees must be up to date to receive service. All charges, including membership fees, will continue to accrue until notice of cancellation is given. Patients may choose to cancel at any time and for any reason by phone, message or mail. Reactivation of an inactive account, either from lack of payment or cancellation will require payment of owed charges and a Re-enrollment fee of $100 per member to re-establish your membership as active. Failure to provide payment will result in our office attempting to contact you via the modes of communication you have selected in the Permission to Share form. If we have been unsuccessful in contacting you, we reserve the right to inactivate your membership.
